When staff are supervising children, they shall always ensure their care, protection, and guidance.
1. On 11/19/2025, Staff #1 fed Child #1, approximately 1 oz of breast milk, from a bottle belonging to Child #2. Child #2 was then fed the remainder of the bottle.

Plan of Correction: Director met with NVR staff and staff #1 to explain implemented new "red sticker" procedures with breast milk bottles as a "stop + check before breast milk is given to ensure it goes to the correct infant. Note: new procedure will be added to the Emp HB for 2025 edition.
