Inspector Notes

The licensing inspector conducted a monitoring inspection in response to a self-report in the area of Programs that was received in the licensing office on 05/02/2023. Interviews were conducted with staff and records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interviews and a record review the center did not ensure physical punishment was not used.
Evidence:
On 05/01/2023 at approximately 3:0pm, Staff #1 used her finger to pluck a 2 year-old child in the forehead for not following directions.
Plan of Correction: An internal investigation was conducted and the staff member was terminated immediately.
