Inspector Notes
The licensing inspector conducted an unannounced monitoring inspection on 06/02/2023 in response to a self report regarding Supervision. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

3Evidence:
On 05/24/2023 at approximately 3:00pm six children, ranging in age from 5 - 9 years-old were not in sight and supervision on the center's van for an unspecified amount of time, when Staff #1 parked in front of a convenience store and left the children in the vehicle. Staff #1 confirmed leaving the children out of sight and sound supervision in a written statement.
Evidence: One of the center's vans had an expired inspection stickers of 2022.
Evidence:
On 05/24/2023 at approximately 3:00pm, staff #1 left six children on the center's van with no other staff present or supervision. It was reported staff #1 told child #2, who was 9 years-old, she was "in-charge," while she left them on the van for an unspecified amount of time, while she went inside a convenience store.
