Inspector Notes
A renewal inspection along with a self -report investigation regarding allegations in the area of supervision, was conducted on 09/05/2023. The self-report A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed 4 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
8Evidence:
1. The last tuberculosis screening for staff #1 was dated 02/21/2021.
2. The last tuberculosis screening for staff #2 was dated 09/02/2021.
3. The last tuberculosis screening for staff #3 was dated 04/16/2019.
Evidence:
In the infant room there was a can of disinfectant spray on the countertop and in the infant/toddler room there was a large bottle of dish detergent on the counter.
Evidence:
1. On 08/30/2023 a 4 year-old child #1 was not in sight or sound supervision of staff for approximately 38 minutes.
2. In a written statement, staff #1 reported she signed herself into the child's class on the Child Supervision Record (CSR) report however, did not conduct a face-to-name check to ensure she accounted for the supervision of all the children.
3. Staff #1 reported she did not know the child was in care until the parents arrived to pick him up for the day and was then found unsupervised in the bathroom with wet and soiled clothing.
Evidence:
There were 2 pacifiers in the infant room that were not individually assigned in any way to identify which baby it belonged to.
Evidence:
In the infant/toddler room a staff member did not wash their hands after changing a child's diaper.
Evidence:
The center had a diaper cream with an expired parent authorization dated 06/23/2023.
Evidence:
The center's record of emergency drills did not have documentation of having practiced a fire drill for the months of May, June, July or August.
Evidence:
Children in the toddler, preschool and three y ear-old classrooms were observed eating lunch. None of the staff in the these classrooms sat with the children during the meal.
