Inspector Notes

A renewal inspection along with a self -report investigation regarding allegations in the area of supervision, was conducted on 09/05/2023. The self-report A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed 4 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
Based on a record review the center did not have documentation that all staff had a repeat tuberculosis screening at least 2 years from the date of the first initial screening.

Evidence:
1. The last tuberculosis screening for staff #1 was dated 02/21/2021.
2. The last tuberculosis screening for staff #2 was dated 09/02/2021.
3. The last tuberculosis screening for staff #3 was dated 04/16/2019.

Based on observation the center did not ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In the infant room there was a can of disinfectant spray on the countertop and in the infant/toddler room there was a large bottle of dish detergent on the counter.

(Repeat Violation) Based on interviews and a record review the center did not ensure children under 10 years of age were always within actual sight and sound supervision of staff.

Evidence:
1. On 08/30/2023 a 4 year-old child #1 was not in sight or sound supervision of staff for approximately 38 minutes.

2. In a written statement, staff #1 reported she signed herself into the child's class on the Child Supervision Record (CSR) report however, did not conduct a face-to-name check to ensure she accounted for the supervision of all the children.

3. Staff #1 reported she did not know the child was in care until the parents arrived to pick him up for the day and was then found unsupervised in the bathroom with wet and soiled clothing.
