Inspector Notes

An unannounced monitoring inspection was completed on 02/04/2025 as a part of the licensure period. The on-site inspection began at 1:15pm and ended at 3:30pm The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 55 children present and 9 staff. The inspector reviewed 5 children?s records and 3 staff record. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

The supply cabinet containing various cleaning products and disinfectant sprays was unlocked in the 3 year-old room.
Plan of Correction: The cabinet was locked during the inspection. The director will remind staff that the cabinets containing chemicals must be locked.
Standard 8VAC20-780-440-B
Cribs, cots, rest mats, and beds shall be identified for use by a specific child.

Child #6 was observed sleeping in a crib that was not identified for specific use
Plan of Correction: This was an oversight. The crib will be labeled in such a way that it identifies specific use by one child.
Standard 8VAC20-780-450-A
Cots used by children other than infants during the designated rest period or during evening and overnight care shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Four children in the preschool room were resting on cots that did not have a bottom cover.
Plan of Correction: Several of the cot sheets have torn and the center was running low. Additional cot sheets will be ordered to ensure all children have both a top and bottom cover during nap.

(REPEAT VIOLATION) Staff shall wash their hands with soap and running water before and after a diaper change.

Staff in the infant room did not wash their hands before or after changing a child's diaper. After changing the diaper, the staff opened cabinets and touched materials before washing her hands.
Plan of Correction: The director will revisit proper diaper changing and handwashing with the entire staff at the next staff meeting. The director will monitor for compliance.

Insect repellent shall be in the original container and labeled with the child's
name.

There was a bottle of insect repellent in the school-age room that was not labeled to identify the name of the child it belonged to.
Plan of Correction: The insect spray was discarded during the inspection. Requirements for the use of insect repellent will be reviewed with staff.
