Inspector Notes
An unannounced monitoring inspection was initiated on 01/28/2025 and completed on 02/04/2025 in response to a self-report received by the licensing office on 01/28/2025 relating to Medication. This inspection included: document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
5On 01/24/2025 Staff A administered an undetermined amount of medication to 3-year-old child #2 that was provided to the center and intended for 5 year-old child #1.The center reported the child's mother verbally told staff A the bottle contained a prescription medication.
Staff A accepted this medication in an unlabeled amber bottle, not in its original packaging and without the parent's signature on the medication authorization form. Staff A did not review the medication authorization form prior to giving child #2 the medication. The center was unable to provide the dosage amount that was given to the child.
01/24/2025 a prescription medication was administered to 3 year-old Child #2 without written authorization from the parent.
The center's medication policy, provided by the Program Director did not include procedures related to duration of the parent's authorization for medication, and methods to prevent use of outdated medication.
On 01/24/2025 Staff A administered a prescription medication to 3 year-old Child #2 that was intended for 5 year-old Child #1. When the medication was received by Staff A from the parent it was not in the original container dispensed from the pharmacy. The medication was in an amber colored bottle with no label and no instructions on dosage, frequency or manner of administration.
On 01/24/2025 Staff A administered a prescription medication to 3 year-old Child #2 and did not keep a record of the medication given.
