Inspector Notes
An unannounced monitoring inspection was completed on 07/22/2025, as a part of the licensure period. The Department requested that the provider complete an Intensive Plan of Correction (IPOC) as result of the last inspection. The center's IPOC was reviewed during the visit. The on-site inspection began at 9:30am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 78 children present and 12 staff. The inspector reviewed 9 children?s records and 4 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-245-J-3
Any child for whom emergency medications have been prescribed shall always be in the care of a staff member or independent contractor who has completed the required training for that specific emergency medication. The center agreed to administer an emergency seizure medication for Child #1, which required rectal administration. There were no staff in the building that had the MAT rectal training required to administer the seizure medication in the event the child needed it.

Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. There was a bicycle on the playground that had a crack with sharp jagged edges on the plastic bike seat.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. The supply cabinet in the Pre-K room was unlocked and contained 2 bottles of disinfectant spray on the bottom shelf accessible to children.

Cosmetics, medications, or other harmful agents shall not be stored in areas that are accessible to children. There was a staff member's personal asthma inhaler in an unlocked cabinet in the Pre-K room in reach of children.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. Five year-old child #1 found an exacto knife on the floor in the Pre-K room and cut his right wrist, which required 3 stiches. Neither staff A nor B responsible for supervising the child saw him with the knife or when he cut himself.
It was reported staff did not follow the center's protocol of checking the exacto knife in and out of the office. Staff reported the knife was stored in the classroom supply cabinet despite being aware that the lock on the cabinet was broken.

Children under 10 years of age always shall be within actual sight and sound supervision of staff. Five year-old Child #1 was not in sight supervision of Staff A and B when he found an exacto knife on the floor and cut the top of his right wrist with the knife. Neither Staff A or Staff B saw the knife or when the child picked it up.

The medication authorization shall be available to staff during the entire
time it is effective. Child #2 had an emergency medication at the center. None of the staff on duty were able to locate the written medication authorization.
