An unannounced focused inspection was conducted on 11/10/2025 as a part of the licensure period. The Department issued a Notice of Intent to place licensee on Probation, mandate training and require the licensee to contact parents, guardians or other responsible persons in writing as a result of high-risk violations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued
