Standard 8VAC20-780-245-A
Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. Staff #1 and #2 did not have documentation of having at least 16 hours of annual training for 2025.
Plan of Correction: The director believes the staff did complete 16 hours if annual training however the documentation was destroyed during flooding and has not been able to collect paperwork from the staff.
Standard 8VAC20-780-280-B
(Repeat Violation) Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. The cabinet over the changing table in the infant room was unlocked and contained a bottle of disinfectant spray.
Plan of Correction: The cabinet was locked during the inspection. The director will remind all staff that chemicals are to be locked when not in use by staff.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts. Staff 4 used her hand to gesture a pluck to a 16 month-old child's face for not follow directions.
Plan of Correction: The staff has been placed on administrative leave. The infant room has been temporarily closed for intense staff training which include behavioral guidance.

It is a forbidden action to put a child in an enclosure in a small confined space or any space that the child cannot freely exit themselves. Staff #1 placed a 16 month-old child #2 in an infant's crib for approximately 25 minutes as punishment.
Plan of Correction: The infant room has been closed so that management can retrain all staff who will work with infants on all standards related to this age group and infant/toddler development. Staff will also be shadowing in other infant rooms within the company. Appropriate behavioral guidance will also be addressed with all staff.

It is a forbidden action to use verbal remarks which are demeaning to a child. Staff #1 used verbally demeaning remarks such as calling Child #2, "bad." It was also reported she has said phrases such as, "she doesn't listen, " I don't like her being here" and "I don't want her in my classroom."

Cribs shall be used for children under 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot or mat. A 16 month-old child #2 who regularly sleeps on a cot in the toddler room, was placed in a crib by staff #1 to keep them contained.
