Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/24/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-520-F
The vendor shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. The list shall be dated and kept confidential in each room or area where children are present.

Child #5 has a dietary restriction. There was not a list with children's allergies, sensitivities, and dietary restrictions in the room where the children were present.
Plan of Correction: Will have on wall by October 31, 2025 other allergies board in hallway.

Within the first 90 days of employment all staff who work directly with children shall complete the Virginia Preservice Training for Child Care Staff.

Staff #1, employed for 16 months, did not complete the Virginia Preservice Training for Child Care Staff.
Plan of Correction: Complete by October 31, 2025.

Staff who work directly with children shall annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course.

1. Staff #1 did not obtain any training hours in 2024 and did not attend the department's annual health and safety update course.
2. Staff #2 obtained 5 hours of training in 2024 and did not attend the department's annual health and safety update course.

Evacuation procedures shall be practiced at least monthly.

The vendor did not practice an evacuation drill in August 2025 or September 2025.
