During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 2/3/2025. The inspection began at 10:00 AM and ended at 12:25 PM. The inspector reviewed compliance in the areas listed above. CDC: There were 30 children present and 6 staff. The inspector reviewed 5 children?s records and 10 staff and corporation member records.. This inspection included:

? interviews, and
? observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on February 14, 2025. A POC submitted after this date will not appear on the public website.
Violations
5Staff 10 did not obtain a tuberculosis screening until six days after employment.
The resilient surfacing (sand) under the flexible seat swings has been kicked out from under the swings, leaving the depth of the surfacing at 2 inches or less under the swings.
Staff 11 was the only staff person on the preschool playground with the children.
The pacifier being used by a toddler on the playground was not labeled with the child's name for individual use.
A crying child in the young preschool classroom was crying and repeatedly wiping his running nose and eyes with his hands. His hands were not washed before playing with the toys.
