During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 2/3/2025. The inspection began at 10:00 AM and ended at 12:25 PM. The inspector reviewed compliance in the areas listed above. CDC: There were 30 children present and 6 staff. The inspector reviewed 5 children?s records and 10 staff and corporation member records.. This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening at the time of employment and before coming into contact with children.
Staff 10 did not obtain a tuberculosis screening until six days after employment.

Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 as shown in Figures 2 (Compressed Loose Fill Synthetic Materials Depth Chart) and 3 (Use Zones for Equipment) on pages 6-7 of the National Program for Playground Safety's "Selecting Playground Surface Materials: Selecting the Best Surface Material for Your Playground," February 2004, and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.
The resilient surfacing (sand) under the flexible seat swings has been kicked out from under the swings, leaving the depth of the surfacing at 2 inches or less under the swings.

When the outdoor activity area is not adjacent to the center, there shall be at least two staff members on the outdoor activity area whenever one or more children are present.
Staff 11 was the only staff person on the preschool playground with the children.

Children's hands shall be washed with soap and running water or disposable wipes after any contact with bodily fluids.
A crying child in the young preschool classroom was crying and repeatedly wiping his running nose and eyes with his hands. His hands were not washed before playing with the toys.
