The licensing inspector conducted an unannounced complaint inspection on 12/07/2023 In response to a complaint that was received in the licensing regarding allegations in the area of supervision. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a record review the provider did not ensure all staff had the required fingerprinting.
Evidence:
1. Staff #1 (hired 1990) had an expired fingerprint check, dated 04/23/2018.
2. Staff #2 (hired 1991) had an expired fingerprint check dated 03/08/2018.

Plan of Correction: The program has a new director who is going through all staff records. Fingerprints will be sent for all staff who do not have them or are expired.
