An unannounced inspection was completed on 07/11/2025 in response to a complaint received by the licensing office relating to allegations of Supervision. inspection included: document review, tour of the facility, interviews, observations and measurements.The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Staff shall be counted in the required staff-to-children ratios only when they are directly
supervising children. Children in the infant room did not have staff directly supervising them during nap. Staff #1 reported during nap she lays on the ground in the infant room and closes her eyes. Staff #1 was shown a picture of herself laying on the infant floor with her eyes closed. Staff #1 confirmed it was her.

Plan of Correction: The picture was taken prior to the current director. The director has only been at the center for 2 weeks. The director will counsel staff #1 on sight and sound supervision and will monitor for compliance.
