During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was conducted on 08/18/2025 from 1:45pm to 3:25pm. There were 34 children and 9 staff present. The inspector reviewed 5 child records and 2 staff records on-site.The inspection included document review, interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business 8/28/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The record for Child #1, enrolled for 7 months, contained an immunizations dated 10 days after the first date of attendance.
Plan of Correction: Child #1 has a current immunization record. Moving forward, new enrollments will have immunization records prior to start date.
Standard 8VAC20-780-500-A
Staff shall wash their hands with soap and running water: before and after a diaper change.

Staff #1 was observed changing 2 children's diapers and did not wash their hands before and after each change.
Plan of Correction: All staff have been retrained on proper hand washing procedures (8-28-2025).
