Inspector Notes
The Licensing Inspector conducted a self-reported announced other inspection on 07/6/2022. The inspector was on site from 12:30pm until 2:45pm. Staff were interviewed and documentation of the incident was obtained. Two children's records and two staff records were reviewed. There were no violations from the self-report, but non-related violations were cited to the program.

Based on review of children?s records, the center failed to obtain 2 designated people to call in emergency if parents cannot be reached.
Evidence: Record for Child #2 did not include the names, addresses and phone numbers for at least 2 people who are not identified as parents/guardians.

Plan of Correction: Child no longer with the program but program will make sure every child has 2 emergency contacts moving forward.
