Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 07/08/2024 and completed on 07/24/2024, as a part of the licensure period. The on-site inspection began at 1:00pm and ended at 5:00pm. The inspector reviewed compliance in the areas listed above. There were 88 children present and 15 staff. The inspector reviewed 7 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/31/2024. A POC submitted after this date will not appear on the public website.
Violations
6Evidence:
1. In the school-age classroom, there was a hole in the wall under the fire extinguisher. This hole had been patched but re-opened.
2. In the school-age boys bathroom, there was water leaking from the ceiling onto the floor.
3. In the school-age classroom, there was an electrical socket that was missing a piece of the cover on the socket.
Evidence:
In the afterschool classroom, there was an unlocked gallon jug underneath the sink with chemicals in it. There were children playing in this area.

In classroom 5, there was food on the shelf with cleaning products.
Evidence:
In classroom #5, there were 7 children on cots with no top sheet.
Evidence:
In the two?s classroom, there was a soiled diaper in an open trashcan in the bathroom.
Based on interview, the center failed to ensure for each group of children, the center shall maintain a written record of daily attendance that documented the arrival and departure of each child in care as it occurs.
Evidence:
1. In classroom #1, there were 7 children present and only 6 children written on the attendance sheet.
2. In classroom #6, there were 8 children signed in on the tadpoles app and 14 children written in on the attendance sheet. There were 16 children present in the classroom at the time of the inspection on 7/8/2024.
