Inspector Notes

An unannounced monitoring inspection was initiated on 9/5/2024 and completed on 9/20/2024 in response to a self-report received by the licensing office on 9/4/2024 relating to physical abuse. The inspector reviewed 1 child?s records and 2 staff records on-site. This inspection included, document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to staff records and are documented on the violation notice issued to the program.

Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.

Staff #1 is currently working and has a TB screening that was dated 3 days after their first day of employment.
Plan of Correction: HR will ensure that all TB?s are done prior to coming into contact with the children 9-23

Documentation of at least 2 references to character, reputation and competency are to be checked prior to employment.

Staff #1, who has been actively working at the center for the past 5 months, had reference checks that were 3 days late.
Staff #2, who has been actively working at the center for the past 2 years, had reference checks that were one day late.
