An unannounced monitoring inspection was initiated on 9/3/2025 and completed on 9/26/2025 in response to a self-report received by the licensing office on 8/26/2025 relating to forbidden Action. The inspector reviewed 1 children?s records and 1 staff records on-site. This inspection included document review, and interviews conducted by phone on 9/23/2025.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
