Inspector Notes

An unannounced monitoring inspection was initiated on December 19, 2023 and concluded on December 28, 2023. There were 39 children present with four staff. Children present ranged in age from five to 11 years old. Children and on-site staff records were reviewed. Children were observed in teacher and self-directed activities including table top activities, large motor games transitions and restroom routines. Violations regarding tuberculosis screening and out of state background checks were cited, appear on the violation notice and were discussed with staff onsite at the conclusion of the inspection.

Based on record review and interview the center failed to ensure that all required background checks are obtained within required time frames.
Evidence:
1 - The record for Staff #3 contains documentation that out of state background checks were not requested until December 28 and 29, 2023.
a. However, the documented date of hire for Staff #3 is November 9,2023.
2 - Staff #1 provided information that confirms the outstate background checks were not requested within required time frames.

Standard 8VAC20-780-160-A
Based on record review and interview the center failed to ensure that each staff member submit a negative tuberculosis screening prior to coming in contact with children.
Evidence:
1 - The record for Staff #2 contains a tuberculosis screening dated October 30, 2023.
a. However, the documented date of hire is October 24, 2023.
b. Therefore, the required screening was obtained six days after Staff #2 came in contact with children.
2 - Staff #1 confirmed that Staff #2 had contact with children prior to obtaining the with children.
