Inspector Notes

A subsidy inspection was initiated on August 26, 2024 and concluded on August 27, 2024. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review the vendor failed to ensure that all staff who work directly with children have completed the required Virginia Preservice Training within required time frames.
Evidence:
