Inspector Notes

An unannounced renewal inspection was conducted on 10/06/2022 from 4:50 pm - 5:40 pm. At the time of entrance there were 14 children in care with two staff. The children in care ranged in age from four years - ten years. A third staff arrived at 5:25 pm. Children were observed in the cafeteria playing with table games, blocks and coloring with markers. Records were reviewed for three children in care.
Five staff records were reviewed at the Blocker YMCA from 2:30 pm - 3:20 pm.

Information gathered during the inspection determined non-compliance with applicable standards and one violation has been documented on the violation notice issued to the program.

Based on record review and interview, the center failed to ensure that children's records contain all of the required elements.

Evidence:
1. Three children's records reviewed lacked documentation of the first day of attendance.
2. Administrative staff confirmed that enrollment dates were not documented on either the paper copy or electronic copy of children's records.
Plan of Correction: Center staff stated that enrollment dates will be added to all children's enrollment records.
