Inspector Notes

A monitoring inspection was initiated and concluded on 03/27/2024. The inspector arrived at 7:35 AM and departed at 8:15 AM . There were 22 children present, ranging in ages from 4 years to school age, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff orientation and emergency contact records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Based upon review of children's records, the facility has not ensured that they have received the required immunizations before the child can attend.
Evidence:
The record provided for child 2 did not include documentation that child 2 has received the required immunizations.

Repeat violation
Based upon observation, review of records and staff interview, the facility has not ensured that a record for each child enrolled is maintained and that each record included all required information.
Evidence:
1. There were 22 children in care with 3 staff during the inspection. Staff 1 stated that three of the children were not enrolled in the YMCA program, however these three children were intermingled with the children in the licensed program and were being supervised by the staff. There were no records available for these three children.
2. The record provided for child 4 did not include information regarding the father/custody.
