During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 5/19/2025, as a part of the licensure period, The on-site inspection began at 7:25am and ended at 8:15am. The inspector reviewed compliance in the areas listed above. There were 13 children present and 3 staff. The inspector reviewed 5 children?s records, and 7 staff emergency contact information were reviewed on-site. Additional staff record was reviewed digitally on 5/30/2025. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 6/19/2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.011-F
The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises. The most recent violation notice was not posted in the center. The document posted was from April 2024. The most recent inspection was completed in December 2024.

The program is required to submit to VDH and implement a plan to test potable water for lead. The program did not have evidence of a testing plan submitted to VDH and/or the program did not have evidence of the implementation of the required testing plan.

Standard 22.1-289.058
Each building that was built before 2015 and that houses a child day program that is licensed pursuant to this chapter or any program described in subdivision A 4, B 1, or B 5 of ? 22.1-289.030 that serves preschool-age children shall be equipped with at least one carbon monoxide detector. Staff were unable to identify the location of the CO2 detector. The facility is licensed for children 4 ? 12 years old.

Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. There are two expired medications in the medication lock box.
