Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #3, employed for more than 4 months, did not have documentation of requesting a central registry check from one state until more than 30 days after the documented date of hire and no documentation of requesting a sex offenders background check from the same state.
Plan of Correction: Going forward center management will ensure that out of state background checks are requested within the required time frames.
Standard 8VAC20-780-200-A
The center shall have a qualified program director regularly be on site at least 50% of the center's hours of operation. During the renewal inspection there was not a qualified program director identified among the staff present. Staff #1 stated that management was in the process of collecting the transcripts of the program director. Staff #2 stated that the Youth Director was rarely present at the center.
Plan of Correction: Going forward center management will ensure that a program director is present 50% of the operation hours. Additionally, a system to document that the program director is present 50% of center's operating hours.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff #4, who has been actively working at the center for the past week, did not have any completed reference verifications on file.
Plan of Correction: Going forward center management will ensure that all references are obtained prior to hire as required.

Staff who work directly with children will attend 16 hours of training and staff development activities to include the department's health and safety update course. The record for Staff #5 did not include documentation that the annual update was completed during the 2025 year based on the staff's date of hire.
