Inspector Notes

An unannounced monitoring inspection was conducted on 3/21/2023. There were 56 children, ages 4 years- 12 years, and 3 staff members present at the beginning of the inspection with an addition staff member arriving during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed in mealtime, and outdoor time. A total of 5 children?s records, 4 staff records and 1 medication were reviewed. The inspector discussed the following with the staff: food brought from home and sanitizing tables. The inspector arrived for the inspection at 4:00 pm and departed at 6:00 pm.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation noticed issued to the provider.

Based on observation, interviews with staff and record review, the center failed to ensure that the required staff to child ratio for children ages 3 years up to school age eligible, 1 staff for every 10 children, shall be maintained.
Evidence:
1. There were 21 children, ages 4 years to 12 years, observed in the purple group with 1 staff member. Staff 3 and 4 and a record review confirmed the age of children. Child 3 was determined to be 4 years old. The required staff ratio for this group of children was 1:10
2. There were 38 children, ages 4 years to 12 years, and 3 staff observed on the playground as one group of children. The required staff ratio for this group of children was 1:10 .
Plan of Correction: Provider will ensure staff maintain proper ratios at all times.
Standard 8VAC20-780-430-K
Based on observation, the center failed to ensure provision shall be made for an individual place for each child?s belongings.
Evidence: Approximately 12 book bags and 8 jackets/coats were observed lying on the floor around the cafeteria table next to the door.
