Inspector Notes

A monitoring inspection was initiated on 03/17/2022 and concluded on 03/21/2022. The director was contacted by email to initiate the inspection. There were 10 children present and 4 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of three staff's records, the center failed to ensure that all out of state background checks were obtained and returned with results within 30 days of employment.

Evidence:
1. Staff # 2 was hired on 9/25/2021 and on the day of the inspection did not have results of the out of state CPS check.
Plan of Correction: The administration in the main office is responsible for obtaining all background checks for the sites. They will monitor all new staff for background checks including out of state and in state checks.
