Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 07/18/2024 and completed on 07/18/2024. The on-site inspection began at 8:15 am and ended at 10:00 am. The inspector reviewed compliance in the areas listed above. There were 28 children present and 5 staff. The inspector reviewed 5 children?s records and 4 staff records onsite. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review and interview with staff, the center failed to ensure that at least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence:
1. The most recent tuberculosis screening for Staff 1 was dated 03/17/2022: there was not a follow-up test/screening for 2024 available for review. Staff 1 confirmed there was no 2024 test/ screening available for review.
2. The most recent tuberculosis screening for Staff 4 was dated 05/18/2022: there was not a follow-up test/screening for 2024 available for review. Staff 1 confirmed there was no 2024 test/ screening available for review.
Plan of Correction: Staff will get screening/test.
