Inspector Notes

An unannounced monitoring inspection was conducted on 11/03/2022. An addition record review was conducted electronically on 11/07/2022. There were 30 children, ages 4 years- 11 years, and 5 staff members providing direct supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, special care provisions and emergencies, special services, and background checks. The children were observed during child-choice activities and outdoor time. A total of 5 children?s records, 5 staff records, and 2 medications were reviewed. The inspector discussed the following with the provider: medication and emergency supplies. The inspector arrived for the onsite inspection at 4:15 pm and departed at 5:45 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observation and interview with staff, the center failed to ensure that children under 10 years of age always shall be within sight and sound supervision of staff.
Evidence: Child 2 was observed entering the room from the playground. Staff 4 confirmed they walked from the playground to the cafeteria out of sight and sound of staff. The entrance from the playground is around the corner and down the hallway from the cafeteria approximately 50 feet.
Plan of Correction: Children will always be in sight and sound of staff.

Based on observation, the center failed to ensure provision shall be made for an individual place for each child?s belongings.
Evidence: The inspector observed approximately 15 book bags lying on the floor next to the door
Plan of Correction: Provider will use alternate means to store individual belongings by 11/10/2022
