Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 6/16/2021 and concluded on 6/16/2021. The director was contacted by telephone to initiate the inspection. There were 49 children present and 10 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on a review of two injury reports, the center did not ensure that complete information was documented for each report. Evidence: The licensing inspector observed that the first injury report reviewed did not contain documentation of future actions to prevent injury from recurring, a second signature from staff or parent, nor the method that a parent was notified. The second report reviewed did not contain a second signature from staff or parent.
Plan of Correction: Administration will ensure that each injury report contains the required information.
