Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on 6/16/2021 and concluded on 6/16/2021. The director was contacted by telephone to initiate the inspection. There were 49 children present and 10 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations

1
Standard 22VAC40-665-520-B-11
Based on a documentation review and administrative interview, the center did not ensure that a written allergy care plan was documented for each child with a diagnosed food allergy. Evidence: The licensing inspector observed the record for Child A indicated a diagnosed food allergy. Child A's allergy was included on the center's allergy list. No allergy care plan was observed in the record for Child A. A member of administration could not locate an allergy care plan documented for Child A during the inspection.
Plan of Correction: Administration will speak with parents and request they provide an allergy care plan from the child's physician.
