Inspector Notes

A monitoring inspection was initiated on May 4, 2022 and concluded on May 4, 2022. There were 51 children present, ranging in ages from infants to preschool with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-770-60-C-2
Based on a review of staff records and interviews, the center did not ensure staff members had a central registry finding within 30 days of employment.

Evidence:
1. The records for staff #5 (start date: 3/28/2022), did not contain the results of a central registry finding.
2. During interview, a member of management confirmed the central registry findings were not received within 30 days of employment for Staff #5. The records did not contain documentation of any further contact.

Based on a review of children's records, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence:
The records for child #1 (date of enrollment 8/25/21) and child #3 (date of enrollment 3/11/21), did not have a physical examination.

Based on observation, the center did not ensure that staff washed their hands with soap and running water after coming a diaper change.

Evidence:
In classroom #3, a staff member was observed changing a diaper. After the staff member changed the diaper, staff member did not wash their hands with soap and running water.

Based on observation and staff interview, the licensee did not ensure the prepared infant formula shall be refrigerated, dated and labeled with the child's name.
