A staff record shall be kept for each staff person that contains documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position.

The record of child #2 and child #3, under the age of two years, did not have an updated immunization record in file.

Diapering area surface shall be cleaned with soap and at least room temperature water and sanitized after each use and disposable diapers shall be disposed of in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

1. In room #2, a staff member did not clean or sanitize the diaper changing surface.
2. In room #2, a staff member did not use a foot-operated trash can and had to use their hands to touch the exterior surface of the trash can to dispose of diapers.

A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
