During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 7/03/2025 and completed on 7/07/2025 in response to a self-report received by the licensing office on 7/02/2025 relating to forbidden actions. The inspector reviewed eight children?s records and five staff records on-site. This inspection included: document review (attendance records, policies and procedures, staff statements, medication authorizations, background checks, training documentation, emergency drills, food, fire, and health permits), tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/24/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. On 6/27/25, Staff #2 and Parent #1 witnessed Child #2 (age 2 years) scribbling on the floor with a marker. They then witnessed Staff #5 make Child #2 clean up the marker with Lysol spray and paper towels.
Plan of Correction: All hazardous materials are locked away
Standard 8VAC20-780-350-B-3
(Repeat/Systemic Violation) The required ratio for children age 2 years is one staff per eight children. The Child Supervision Record for Classroom #3 documents Staff #2 as being present from 10:00 a.m.-6:30 p.m. along with Staff #5. Staff #2 was also documented as being in Classroom #2 from 7:00 a.m.-1:48 p.m. and 2:36 p.m.-6:45 p.m. When the Child Supervision Record for Classroom #3 was reviewed with Staff #2, she saw that her name and initials were on the form but stated that it wasn't her handwriting. Staff #2 could not be in two classrooms at the same time, the CSR for Classroom #3 documents that at 10:15 a.m. there were 15 children with Staff #5. At 12:30 p.m. there were 16 children with Staff #5 and at 4:00 p.m. there were 14 children with Staff #5.

(Repeat/Systemic Violation) The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards and the center's own policies and procedures that are required by these standards.

1) The Center?s ?Behavior Guidance Plan For Parents? states that ?We will offer alternatives to children engaged in undesirable behavior by offering a different toy, suggesting a new activity, engaging the child in an activity with a teacher or another child, or by suggesting independent play? and that ?the teacher helps the child to understand the logical consequences of his/her actions by removing an object, activity etc. that is causing the problem.? The ?Behavior Guidance Plan For Parents? also prohibits corporal punishment. On 6/27/25, Child #2 scribbled on the floor with a marker. Staff #2 and Parent #1 witnessed Staff #5 spank Child #2 before making Child #2 clean up the marker with Lysol spray and paper towels. Staff #5 then spanked Child #2 again.

2) The Center?s ?Child Supervision Record, Quick Guide? requires staff to ?Write the exact times for arrive (sic) and departure.? The Child Supervision Record for Classroom #2 dated 6/27/25 does not document the departure time for three children. The Child Supervision Record for the Classroom #6 dated 6/27/25 does not document the departure time for seven children.

3) The Center?s ?Child Supervision Record, Quick Guide? requires that when a staff member ?enters ratio they must sign in with their first and last name, noting the arrival time. When they leave, for any reason, including break or leaving for the day they must sign out with the corresponding time.?
A) Two staff members did not sign out for breaks on the Child Supervision Record for Classroom #1 dated 6/27/25.
B) One staff member did not sign out when she left for the day on the Child Supervision Record for Classroom #2 dated 6/27/25.
C) One staff member did not sign out for breaks on the Child Supervision Record for Classroom #3 dated 6/27/25.
D) Three staff members did not sign out for breaks on the Child Supervision Record for Classroom #4 dated 6/27/25 and two staff members did not sign out when they left for the day.
E) One staff member did not sign out for breaks on the Child Supervision Record for Classroom #5 dated 6/27/25.
F) One staff member did not sign out for breaks on the Child Supervision Record for Classroom #6 dated 6/27/25.

4) The Center?s ?Child Supervision Record, Quick Guide? requires that ?Management observe at least two Name-to-Face and/or transition processes per day, document accordingly.?
A) Management did not document observations of any Name-to-Face and/or transition processes on the Child Supervision Records for Classrooms #1, #4, #5, and #6 dated 6/27/25.
B) Management did not document two observations of Name-to-Face and/or transition processes on the Child Supervision Record for Classroom #2 dated 6/27/25.

5) The Center?s ?Child Supervision Record, Quick Guide? requires that staff ?Document the number of children and teachers at the bottom of the CSR? and ?Teacher(s) should initial after each Name to Face Transition and Name to Face Check.? The initials of Staff #2 and Staff #5 were written on the Child Supervision Record for Classroom #3 for each name to face check on 6/27/25. When the CSR for Classroom #3 dated 6/27/25 was reviewed with Staff #2, she stated that they weren?t their initials. Staff #2 was documented as being in Classroom #3 and Classroom #2 at the same time between 10:00 a.m.-1:48 p.m. and between 2:36 p.m.-6:30 p.m.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts. On 6/27/25 Child #2 (age 2 years) scribbled on the floor with a marker. Staff #2 and Parent #1 witnessed Staff #5 spank Child #2 before making Child #2 clean up the marker with Lysol spray and paper towels. Staff #5 then spanked Child #2 again. The response of Staff #5 was not constructive or age and stage appropriate.
