During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on 7/03/2025 and completed on 7/07/2025. The on-site inspection began at 10:45 a.m. and ended at 4:00 p.m. on 7/03/2025. The inspector reviewed compliance in the areas listed above. There were 68 children present and 14 staff. The inspector reviewed eight children?s records and five staff records on-site. This inspection included document review (attendance records, policies and procedures, staff statements, medication authorizations, background checks, training documentation, emergency drills, food, fire, and health permits), tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/24/2025. A POC submitted after this date will not appear on the public website.

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #2 (age 2 years) has been attending the center for more than 15 months. There were no updated immunizations in the record for Child #2

Training on the center's emergency preparedness plan shall be completed annually and each time the plan is updated. There was no documentation that staff received training on the center's emergency preparedness plan within the last year.

Medication shall be kept in a locked place using a safe locking method that prevents access by children. The emergency medication for Child #7 was not kept in the locked medication box as required with the rest of the children?s medication.

One working, battery-operated radio in each building used by children is required. Staff #6 was unsure if they had a radio or not and was unable to find a radio during the inspection.

Any vehicle used by the center for the transportation of children shall meet the safety standards set by the Department of Motor Vehicles and shall be kept in satisfactory condition to assure the safety of children. Van 17189 had a registration that expired in 5/2025. Staff #6 reported that the van was used for a field trip on 7/03/2025.

The center shall ensure that Emergency numbers, the center's name, address, and phone number, a list of the names of the children being transported, and allergy care plans and information is in transportation vehicles during transportation of children. The required information was not in Van 17189. Staff #6 reported that the van was used for a field trip that day.
