Staff shall complete orientation training that includes all of the required topics prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. Staff #3 completed orientation training more than 3 months after her start date.
Plan of Correction: Will make sure that staff sign off on orientation by the first day of work in the future.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. The vents in the ceiling panels throughout the center were observed to be dirty, caked with dust and lint.
Plan of Correction: Work order was placed and an updated one was placed on 11/03/25.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff #3, who has been actively working at the center for the past 4 months, did not have any completed reference verifications on file.
Plan of Correction: References were obtained by our 3rd party vendor, HireRight. Will make sure to print a copy and file when completed in the future.
