Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program.

A self report inspection was initiated on 10/13/2021. The director was contacted by telephone , interviews and documents were reviewed and inspection was conducted.

The evidence gathered during the investigation supported the non-compliance with and violations were issued. Any violations during the course of the investigation can be found on the violation notice.

Based on staff interview and documentation, the center did not ensure to keep all areas free of obvious injury hazards.

Evidence:
Staff#1 contacted the licensing inspector on October 13, 2021 of a incident that occurred on August 27, 2021. Staff #2 had a carabiner attached to a lunchbox with keys and pepper spray. Child #1 sprayed pepper spray in his face . Staff #1 and Staff #2 confirmed the pepper spray was not kept in a locked place. The center called 911 for medical assistance.
Plan of Correction: All staff members personal belonging that contain lighter, medication or any hazardous material will be kept in a locked cabinet. Spoke to all staff immediately after incident happen individually on August 30, 2021 and August 31, 2021 and during a all staff meeting on September 14, 2021.
