Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was completed on September 29, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were thirty-nine children present, ranging in ages from infants to four years, with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review and interview, facility did not ensure 1 of 3 staff have an updated TB screening every 2 years.

Evidence: The record for staff #1 had a TB screening/test dated 3/12/2019 which is more than 2 years old. Director confirmed this is the most recent results.
Plan of Correction: Staff is scheduled to have the screening/test completed next week.

Based on review and interview, facility did not ensure required background checks are updated every 5 years.

Evidence: The record for staff #1 had a most recent Central Registry Search dated 12/21/2015 which is more than 5 years old. Director confirmed this is the most recent Central Registry Search completed.
Plan of Correction: Director will have the Central Registry Check completed and signed by staff. The check will be mailed tomorrow.
