Inspector Notes

A monitoring inspection was initiated on March 8, 2023 and concluded on March 13, 2023. There were forty-five children present with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review, facility did not ensure required out of state background checks are completed for staff that have lived outside the state of Virginia within the past 5 years.

Evidence: The record for staff #4 (start date 11/28/2022) did not have a completed child abuse and neglect registry search for the state the individual has lived in within the past 5 years.
Plan of Correction: The background check will be completed and submitted. In the future, out of state background checks will be completed as required.
Standard 8VAC20-780-570-A
Based on observation and interview, facility did not ensure children have a protective belt fastened securely when sitting in a high chair.

Evidence: In the infant room, 1 of 4 infants observed eating in a high chair did not have a protective belt securely fastened. Staff stated that the belt did not work.
Plan of Correction: High chair will be removed from the classroom.
