Inspector Notes

An unannounced monitoring inspection was conducted on June 5, 2023 from approximately 9:30 am to 11:40 am. Forty-seven children were present with eight staff supervising. Additional staff were on site for administrative and kitchen duties. Children were observed participating in various learning activities. Four staff and five child records were reviewed. Violations found are noted on the violation notice.

Standard 8VAC20-780-330-B
Based on observation, facility did not ensure there is required resilient surfacing under and around climbing equipment and equipment with moving parts in the fall zones on the playground.

Evidence: The following was observed:
1. There is approximately 4" depth of sand at the foot of the blue slide on the toddler playground.
2. There is approximately 3" depth of sand under the swings on the toddler playground.
3. There is approximately 3" depth of sand at the foot of the green and brown plastic slide on the side of the larger center playground.
Plan of Correction: A load of sand will be ordered today.
Standard 8VAC20-780-80-A
Based on observation and review, facility did not ensure 3 of 5 classrooms have a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: The following was found:
1. In the 2's room, 9 children were observed however the attendance showed 7 children present. .
2. In the 3's room, 9 children were observed however the attendance showed 10 children present.
3. In the 4's & 5's room, there was no documentation of attendance for the group.
Plan of Correction: Director will talk with staff present today about the importance of accurate attendance as children arrive and depart each group. Staff not present today will be told upon their return. A policy will be written and signed by all employees at the next staff meeting.
