Inspector Notes

An unannounced monitoring inspection was conducted on October 30, 2023 from approximately 12:30 pm to 2:20 pm. Seven staff were present supervising forty-seven children ranging in age from infants to five years. Additional staff were on site for administrative and kitchen duties. Children were observed eating lunch, napping and playing freely. Four staff and five child records were reviewed. Violations were found that are noted on the violation notice.

Based on observation, facility did not ensure areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

Evidence: The following was observed during inspection:
1. Portions of the wood border on the path leading to the infant toddler playground are decaying with missing sections and splintered edges.
2. A red plastic tricycle seat was broken with missing section leaving sharp edges.

Based on observation, facility did not ensure there is sufficient resilient surfacing in fall zones under and around play equipment on the playground that have moving parts or are climbing equipment.

Evidence: A green and brown plastic slide was observed sitting on approximately 2" depth of sand outside the bordered play structures on the playground.
