During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/16/2025 and completed on 10/1/2025. The on-site inspection began at 1:50 pm and ended at 5:15 pm. The inspector reviewed compliance in the areas listed above. There were 35 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records on-site on 9/16/2025. This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The programis required to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. The record for staff #4 did not contain results of a sex offender registry search for one state where the employee resided within 5 years of hire. The results of the sex offender registry search were more than 2 years overdue.
Plan of Correction: The Out of Sate Sex offender registry search was conducted during the inspection and results were placed in staff #4's record.
Standard 8VAC20-780-260-A
The center is required to obtain an annual fire inspection report from the appropriate fire official having jurisdiction. The last fire inspection conducted was dated 7/13/2025. The annual fire inspection is dated 7/13/2025 and is overdue by 2 months.
Plan of Correction: The fire inspector was contacted during the inspection and will be inspect ing the facility in 3 days.
