Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on March 23, 2021 and concluded on March 25, 2021. The in-charge person was contacted by telephone to initiate the inspection. There were eight children present and four staff. The inspector emailed the administrative staff a list of items required to complete the inspection. The Inspector reviewed two children?s records and no staff records (all staff records had been previously reviewed) during a virtual face-to-face inspection to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on interview with administrative staff, the center failed to ensure that there was annual approval from the local health department.

Evidence:
1. The last health report form completed for the center was dated 7/29/2019. The administrative staff person in charge of having the health inspection and reports completed stated that she does not recall a health inspection being completed in the year 2020.
Plan of Correction: An administrative staff person will contact the health department to have an inspection and form completed.
