Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including phone contacts, documents review, and a virtual tour of the program.
A monitoring inspection was initiated on 02/11/22 and concluded on 02/22/22. The director was contacted by phone and a virtual inspection was conducted. There were 12 children present, ages three and four, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and one new staff record was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards of law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Based on record review, the facility failed to ensure that prior to employment or volunteering, all persons who are left alone with children have completed fingerprint background checks.
Evicence:
1. The last fingerprint check for staff #1 was dated 02/14/2018. This person was rehired 01/28/2022.
Plan of Correction: There is a new HR Manager who is reviewing records and contacting the IT department to collect needed information. A new fingerprint will be completed if the most recent fingerprint check was not completed or found.
Standard 8VAC20-780-160-A-1
Based on documentation review, the center failed to get a TB screening at the time of employment and prior to coming into contact with children.
Evidence:
1. Staff #1 had documentation of a TB screening dated 02/14/2018 and the hire date is 01/28/2022.
Plan of Correction: There is a new HR Manager who is reviewing records and contacting IT to see what information can be retrieved .
Standard 8VAC20-780-240-C
Based on documentation review the center failed to have orientation training appropriate to the age of children in care with all the required facility topics.
Evidence: The inspector reviewed staff person's #1 orientation form and it did not contain the required updates that went into effect 10/13/2022.
Plan of Correction: A copy of the updated standards have been received and necessary changes will be made in the orientation procedures and program policies. HR and managers have been informed the new changes to orientation.
