Inspector Notes

An unannounced monitoring inspection was conducted on 3/23/2023 and concluded with an electronic record review on 3/29/2023. There were 12 children, ages 3 years- 4 years, and 3 staff providing direct supervision and support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed in self-directed activities and outdoor time. A total of 5 children?s records, 4 staff records and 2 medications were reviewed. The inspector discussed the following with the staff: hand washing and attendance. The inspector arrived for the inspection at 9:30 am and departed at 10:45 pm.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation noticed issued to the provider.

Based on record review, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia.
Evidence: The record for Staff 4 contained a documented hire date of 06/07/2021. The employee eligibility letter was dated 06/09/2021.
Plan of Correction: Provider will obtain required information before hiring employees.

Based on record review, the center failed to ensure that an employee must not be employed until the agency has the person?s completed sworn statement or affirmation.
Evidence: The record for Staff 4 contained a documented hire date of 06/07/2021. The sworn statement was dated 06/08/2021.
Plan of Correction: Provider will obtain required information before hiring employees.
Standard 8VAC20-780-540-E
Based on observation, the center failed to ensure the following non-medical emergency supplies shall be required: one working battery-operated radio in each building used by children.
Evidence: During the inspection, a working battery-operated radio was not available.
Plan of Correction: Staff will obtain batteries.
