During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 1/28/2025, as a part of probation. The on-site inspection began at 11:58am and ended at 12:35pm. The inspector reviewed compliance in the areas listed above. There were 48 children present and 12 staff. The inspector reviewed 5 children?s records and 2 staff records on site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (date). A POC submitted after this date will not appear on the public website.
Violations
2A nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing.
Room 4 had a diaper changing pad with a large cut exposing the soft, absorbent cushion inside.
Documentation shall be maintained of emergency evacuation, shelter-in-place, and lockdown drills that includes: Identity of the person conducting the drill; The date and time of the drill; The method used for notification of the drill; The number of staff participating; The number of children participating; Any special conditions simulated; The time it took to complete the drill; Problems encountered, if any; and For emergency evacuation drills only, weather conditions.
The facility did not have complete documentation of the emergency drill log.
