Inspector Notes

An onsite renewal inspection was initiated and concluded on October 4, 2022. There were 30 children in care ages 24 months through three-years-old. There were seven staff present including the owner and administrators. Children's and staff records were reviewed. Children were observed in teacher and self-directed activities including playing with toys, transitions, hand washing, story time, lunch and nap. Violations regarding attendance and ratios were observed, discussed with the program director during the exit interview and appear on the violation notice.

Based on observation and interview the center failed to ensure that there is one staff every 10 children ages 3 years to school age.
Evidence:
1 - At approximately 10:45am there were 11 three-year-old children in care with one staff where two staff needed.
2 - Staff #2 stated that there were 11 children present in the classroom and she was the only staff present caring for all 11 children.
Plan of Correction: A mistake was made with the attendance sheet and the 11th child was overlooked. The mistake was corrected during the inspection and a child was moved to another classroom to bring the total number children in the classroom back down to 10.

Based on observation and interview the center failed to ensure that each group children maintained a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1 - In the three-year-old, at approximately 10:45am the attendance sheet indicated that there were 10 children present. However, there were 11 children present in classroom.
2 - Staff #2 stated that Child #1 arrived at 9:30am and she had not written his name on the attendance sheet.
Plan of Correction: Staff forgot to list the child as he arrived. The mistake was corrected during the inspection and the child's name and time he arrived was added to the attendance sheet.
