Inspector Notes

An unannounced monitoring inspection was initiated on October 28, 2023 and completed on August 19, 2024 in response to a self-report received by the licensing office on October 28, 2023 relating to Programs. The inspector reviewed one child?s records, and one staff record electronically on August 28, 2024. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on .October 23, 2024. A POC submitted after this date will not appear on the public website.

Staff will ensure that the action or threat of roughly handling is forbidden.
Child #1 is seen on video footage as Staff #1 yanks the child up off the cot by one arm and tosses him back down on the cot twice. Staff #1 then yanks a blanket off the child causing his legs to swing up in air, then replaces the blanket on top of the child before walking away.
