Inspector Notes

A monitoring inspection was initiated and concluded on 04/11/2024. The inspector arrived at 7:10 AM and departed at 7:45 AM. There were 12 school aged children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 staff site records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview. The program director provided the plan of correction for the violation cited.

Based upon review of written injury records, the facility has not ensured that the written records of children's injuries include all required information.
Evidence:
1. An injury record dated 9/28/23 for child 6 did not include the time the parent was notified of the injury.
2. An injury record dated 10/9/23 for child 7 did not include the date and time the parent was notified of the injury.
3. An injury record dated 10/27/23 for child 8 did not include the time the parent was notified of the injury.
