Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Staff 1 began employment on 10/8/2025. There was no documentation that staff 1 had received the required orientation training.
Plan of Correction: Facility response:
All new staff will complete orientation within seven days of hire. Staff 1 competed orientation and documentation has been added to the binder.
Standard 8VAC20-780-40-M
The center shall maintain a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan.
The allergy list for the facility was not current. A child with a severe peanut allergy )child 7)was listed, however the child was no longer enrolled at the facility.

The allergy list has been updated to remove the unrolled child and now reflects only current participants

The center shall maintain a written record of children's serious and minor injuries that includes all required documentation.
Two injury records dated 11/3/2025 did not include the dates, times and how the parents were notified of the children's injuries.
An injury records dated 10/3/2025 and 10/21/2025 did not include the dates the parents were notified of the children's injuries.
Plan of Correction: facility response
All injury reports will be completed in full, including the date, time, and method of parent notification. The missing parent notification details for the listed injury reports have been corrected where possible. Staff have been retrained on competing injury documentation.
